Endocrine Adrenal Tumors Koreans

Abstract
|
|
|
Endocrine adrenal tumors are clinically very important becaue most of them are curable by appro priate therapy, but they are relatively uncommon in Korea, probably due to lack of attention. We analyzed the clinical, biochemical, radiological findings and the results of therapy of 117 patients with endocrine adrenal tumors who visited Seoul National University Hospital from 1962 to Feb. 1984.
The results are as follows:
1) In the 59 patients with Cushing´s syndrome, 33 had Cushing´s disease, 17 adrenal adenoma, 3 carcinoma, 4 nodular hyperplasia and 2 had ectopic ACTH syndrome. Unilateral adrenalectomy was performed in the 17 patients with adrenal adenoma and 3 with carcinoma. In the 37 patients with Cushing´s disease, 18 underwent bilateral adrenalectomy, 4 pituitary irradiation and 4 transsphenoidal hypophysectomy.
2) In the 7 patients with adrenogenital syndrome, 5 patients had congenital adrenal hyperplasia, 1 adenoma, 1 carcinoma. All patients were females and showed generalized masculinization and/or anomalies of sex organs. Patients with congenital adrenal hyperplasia were treated with glucocorticoid, and remarkable symptomatic improvement were noted.
3) In the 19 cases of primary aldosteronism, 11 had adenoma(l bilateral), 2 bilateral adrenal hyperplasia, 1 nodular hyperplasia and 1 carcinoma. Significant improvement in blood pressure and serum potassium levels were noted in 10 of 11 patients with adenoma who underwent adrenalectomy.
4) In the 32 cases of pheochromocytoma, 21 had adrenal adenvma(2 of them bilateral) and .. 2 had adrenal medullary hyperplasia. Eight cases arose from the extr adrenal chromaffin tissues including 1 case from- the urinary bladder and 1 from the glomus jugulare. One patient had both adrenal and extraadrenal pheochromocytoma. 29 patients underwent surgical intervention, and 28 of them showed remarkable improvement in blood pressure and biochemical data.
